Award Criteria
- Demonstrated impact of (formal or informal) leadership roles/positions
- Long term contributions to CPD
- Demonstrated impact of innovation(s)
- Demonstrated sustained commitment to and excellence in education (which may include teaching, program coordination, administration, and/or scholarly activities in CPD)
Eligibility
- All faculty (affiliated with University of Toronto Faculty of Medicine) need to have a minimum of 10 years of academic and/or clinical experience for consideration

Required Submission Documents
- Executive summary that addresses the award criteria, outlining the nominee’s accomplishments, and the nominator’s reasons for supporting the nomination. The focus of the summary should be on work in Continuing Professional Development including research/scholarly activity, leadership/administrative roles, and education. The summary should provide evidence of the demonstrated impact of these endeavours, and of sustained commitment over time in the field of CPD
- A letter of nomination from a departmental chair, departmental CPD director, or designate (the executive summary can be embedded within this letter)
- Up to date CV highlighting CPD roles (Include: Grants, publications, invited lectureships, mentorships, chairmanships, directorships etc.)
- An additional two letters of peer support (emails also accepted)
Optional Submission Documents
- Material or data that supports the nomination and highlights the commitment, impact and successes of the nominee. Evaluation data can be included, but please do not send raw data. Summaries or aggregated data are preferred

David Fear Fellowship awarded to a Faculty of Medicine faculty member to develop or attend a continuing professional development program

Fred Fallis Award in Online Learning recognizes an individual or group which has demonstrated innovation and excellence in online learning for health professionals

Excellence in Interprofessional CPD Award recognizes excellence in CPD activities designed to enhance interprofessional team practice and improve healthcare delivery

Ivan Silver Innovation Award recognizes excellence in innovation in a CPD initiative developed and delivered by a University of Toronto faculty member or team
Awards Deadline
Deadline for nominations for the previous academic year is November 30 (If November 30 falls on a weekend the deadline will be the following working day).
