All registration fees are subject to 13% HST
*Proof of status in the form of official letter on institutional letter head must be provided at time of registration.
**Proof of status in the form of a photo of ID showing name and date of birth must be provided at time of registration.

Indigenous Health Conference Fund

Would you like to assist an Elder or Indigenous Undergraduate Student attend the conference? Donations to offset the cost of registration can be added to your conference registration fee in the amount of $50, $100, $150. For amounts over $150, please contact: facmed.registration@utoronto.ca. Please reference fund name: CPD Event Support, c/o Indigenous Health Conference Fund, Faculty of Medicine.

Note: You will receive a tax receipt for the amount of your donation (excluding your conference registration fees). May take 4-6 months post-event.

Bursary Application

Indigenous Elders and Undergraduate Students wishing to apply for registration bursaries may complete the application below:

The number of bursaries offered will depend on the amount of donations received and will be announced on Monday, November 16.

Cancellation

Cancellations will be accepted until Thursday, November 19, 2020 and are subject to a processing fee of $60 plus applicable taxes. Refunds will not be processed after this date.

Requests for cancellation must be made in writing to facmed.registration@utoronto.ca. Registrations are not transferable.

The University of Toronto reserves the right to cancel events. Registrants will be notified at the earliest possible date in the event of a cancellation. Registration fees for events cancelled by the University will be refunded; however, the University will not be liable for any loss, damages or other expenses that such cancellations may cause.
