Dermal Regeneration Matrices (DRMs) are increasingly becoming an essential part of acute burn care1 and scar reconstructive procedures. They have demonstrated an improvement to both long-term functional and cosmetic results and thus, enhance the quality of life for patients having undergone both acute and reconstructive burn surgeries. Although there is an emerging body of evidence showing the need of DRMs for acute and reconstructive burn surgery, further research is warranted, not only to strengthen scientific evidence regarding their effects, but also to promote the development of new techniques, such as single stage reconstruction, and technologies, such as scaffolds for stem cells, tissue engineering, and regenerative medicine products.

Matriderm Dermal Matrix is a single use, three-dimensional acellular matrix composed of structurally intact bovine collagen fibers and hydrolyzed elastin for tissue regeneration. Originally developed in Europe, Matriderm has 14 years of successful applications for the treatment of acute burns, reconstruction of burn injuries, and treatment of traumatic defects. The device has been commercially available since January 2004 in Europe and other countries around the world. Health Canada has recently approved MatriDerm for sale in Canada.
Matriderm allows for the advanced one-step repair of partial and full-thickness defects. When applied with an autologous split-thickness skin graft, Matriderm is transformed into a highly functional tissue that closely resembles intact human skin. When used together, there is less donor site morbidity, improved plasticity, improved elasticity, decreased wound bed contraction, and restored optimal functioning over demanding areas.

Martin C. Robson, MD, received his MD from the Johns Hopkins University. He completed his general surgery residency at the Brooke Army Medical Center and his plastic surgery residency at Yale. He has served as Chief of Plastic Surgery at the University of Chicago, Wayne State University, and the University of Texas Medical Branch. He is presently an Emeritus Professor at the University of South Florida. He is a Fellow of the American College of Surgeons and holds Honorary Fellowships from the Royal College of Surgeons of England and the Royal Australasian College of Surgeons. He has served as President of the American Burn Association and received its Distinguished Service Award. He has been President of the Wound Healing Society and is a recipient of its Lifetime Scientific Achievement Award. He has also received Lifetime Scientific Achievement Awards from the Association of Advanced Wound Care and the World Union of Wound Healing Societies. He has been Chairman of the American Board of Plastic Surgery, Chairman of the Residency Review Committee for Plastic Surgery, Chairman of the Plastic Surgery Research Council, and President of the Association of Academic Chairmen of Plastic Surgery. He has authored over 600 publications mostly devoted to wound healing, wound infection, burns, and proliferative scarring.

An intensive didactic and hands on course exclusively offered to Resident & Faculty Plastic surgeons lead by an expert surgeon, Dr. Shahriar Shahrokhi, to examine the clinical use of IDRT in acute and scar reconstruction procedures, review patient selection criteria and surgical pearls for successful application of IDRT. Didactic presentations will be followed by an opportunity for hands on debridement techniques and application of IDRT.

The approach and management of burn therapy has transitioned with the introduction of the Fractional CO2 Laser System and SCAAR FX technology. Burn centers across the US and the world have adopted this treatment for their patients with outstanding results and life-changing outcomes. The ability to effectively and non-invasively re-model tissue with the newer generation CO2 laser has been revolutionary. Introduction of this technology earlier in the treatment process has resulted in benefits such as improved blood flow to enhance sweat, proper hair growth, improved mobility and range of motion, reduction of secondary wounds, reduction in pain, tightness and scar itch, improved cosmesis/return to normalcy and improvement of scar texture. Treatment over and around skin grafts has also proven to provide excellent results for patients. The implementation of SCAAR FX into a hospital burn center and the algorithms associated with the therapy will be discussed along with clinical examples and patient outcomes.

Matthias B. Donelan, Shriner’s Hospital for Children Boston, MA
Matthias B. Donelan, M.D., is the chief of staff for Shriners Hospitals for Children — Boston and is board certified by the American Board of Plastic Surgery. He received an undergraduate degree in biology from Harvard College in Boston and earned a medical degree from the Tufts University School of Medicine Dr. Donelan completed his residencies in General Surgery and Plastic Surgery at Massachusetts General Hospital. He has also completed a student fellowship in pathology at New England Medical Center in Boston; and was the Tutor Specialist in Plastic Surgery at Middlemore Hospital, New Zealand.
Dr. Donelan is a widely recognized expert in the field of burn reconstructive surgery and has developed numerous innovative techniques to enhance the care of burn patients He has multiple publications in peer-reviewed scientific journals and has written definitive textbook chapters on burn reconstruction. Dr. Donelan has long been an advocate for scar rehabilitation through tension relief and the use of the pulsed-dye laser. He is currently investigating fractional CO2 laser treatment for aesthetic and reconstructive indications in burn and trauma patients. In addition to clinical and scientific activities he is involved in residency training and is on the executive committee of the Harvard Combined Plastic Surgery Training Program.
